Our Pledge to You

ElinMed is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, and abide by the terms of this Notice. We are committed to protecting your health information and ensuring you understand your rights.

How We May Use and Disclose Your Health Information

For Treatment

We use your health information to provide, coordinate, and manage your cardiac care. For example, Dr. Nyange may use your medical history during a consultation, order labs, or coordinate with another specialist.

For Payment

We may use and disclose your health information to bill and collect payment for services. For example, we may provide information to process your payment or prepare a superbill.

For Health Care Operations

We may use your health information for internal operations such as quality assessment, training, and compliance reviews.

Other Permitted Uses

Public health activities required by law
Research (with proper authorization and safeguards)
Compliance with legal and regulatory requirements
Emergency situations to prevent serious harm
Communications with family or friends you authorize

Uses Requiring Your Authorization: We will not use or disclose your health information for marketing purposes, the sale of your health information, or psychotherapy notes without your written authorization.

Your Rights

Right to Access Your Records

You may request to inspect and receive a copy of your health information. Submit a written request to privacy@elinmed.com. We will respond within 30 days.

Right to Request Amendment

If you believe information in your records is incorrect or incomplete, you may request an amendment. We may deny the request with written explanation.

Right to Request Restrictions

You may request restrictions on how we use or share your health information. We are not required to agree to all requests, but if we do, we will honor them except in emergencies.

Right to an Accounting of Disclosures

You may request a list of certain disclosures made of your health information within the past six years.

Right to Confidential Communications

You may request that we communicate with you in a specific way or at a specific location (e.g., only by email).

Right to a Copy of This Notice

You may request a paper copy of this Notice at any time by contacting us.

Right to File a Complaint

If you believe your privacy rights have been violated, contact our Privacy Officer or file a complaint with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.

Our Safeguards

ElinMed uses physical, technical, and administrative safeguards to protect your information, including encrypted data transmission and storage, secure electronic health records systems, limited access on a need-to-know basis, and regular staff privacy training.
